  • Registered Nurse per Diem

    Reports to: Administrator

     

    Job Responsibilities:

    • Work to develop, implement and evaluate the plan of care and make necessary revisions to address all problems in the plan of care.  Hospice nurse provides coordination of the plan of care, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional allied services.
    • Provide education, supervision and counseling to the client and their family regarding hospice procedures and other care needs as appropriate to client’s needs.  Hospice nurse assures that the client and their family is involved in the development of the plan of care whenever possible.
    • Training is provided on electronic charting/documentation of patient information; however basic computer skills are required
    • Responsible for creation of time sheets and other required patient/employment documents & reports.
    • Visits are made in home environments, assisted living facilities, nursing homes….wherever the patient resides.

    Job Requirements:

    • Must be able to stand for long periods of time,  lift a minimum of 30 pounds.
    • Current RN Illinois state license in good standing
    • At least one (1) year recent experience in the delivery of hospice nursing services
    • Background check & drug test will be administered
    • TB test, Hepatitis B & physical provided by the employer
    • Must have reliable transportation, a valid driver’s license, and the minimum state required liability auto insurance
  • On Call Nurse

    Reports to: Administrator

     

    Job Responsibilities:

    Responsible for providing skilled nursing, intervention, and supportive care to patients in the homesetting for evening, night, and holiday on-call duty in accordance with the Apex Hospice and Palliative Care Interdisciplinary Plan of Care for each patient.

    Job Functions:

    • Assume responsibility for evening, night, and holiday on-call duty as scheduled and consistent with Apex Hospice Care policies and procedures.
    • Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
    • Assume primary responsibility for Apex Hospice and Palliative Care census during assigned shift, including assessing, planning, implementing and evaluation phases of the nursing process.
    • Maintain regular communication with the Nurse Supervisors, attending physician, and case managers concerning patient/family care needs and response to interventions.
    • Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
    • Respond to medical or emotional crisis and/or death of patient and make home visit to provide direct care as appropriate. Provide appropriate support at time of death.
    • Maintain up-to-date patient records so that problems, goals and interventions are accurately and clearly stated and changes are reflected as they occur.
    • Documentation in the Electronic Medical Record System is completed within agency guidelines and to meet regulatory requirements.
    • Educate primary caregivers, families, volunteers, and employed caregivers to provide care for patients as indicated.
    • Meet regularly with hospice nursing staff to review problems or unique issues from on-call hours, share professional support, and exchange feedback aimed toward enhancing professional growth.
    • Attend Interdisciplinary Team meetings and facilitate discussion of issues from on-call hours for full staff discussion, consultation and evaluation.

     

    Qualifications:

    • Possess a current license to practice as a registered professional nurse in the State of Illinois
    • Experience as a professional nurse; with experience preferably in an acute, oncology, Home Health and/or Hospice setting.
    • Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
    • Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
    • Ability to turn, reposition, and assist patients to transfer and ambulate
    • Must be able to bend, stoop, push and pull
    • Must be able to lift a minimum of 35pounds
    • Requires manual dexterity sufficient to provide hands-on care to the patient under variable conditions and settings
    • Possess a valid Illinois driver’s license, valid auto insurance, and an automobile for field use
    • Background check will be administered
    • TB test, Hepatitis B & physical provided by hospice
    • Drug test will be administered
    • Must have reliable transportation, a valid driver’s license, and the minimum state required liability auto insurance.
  • Social Worker per Diem

    Reports to: Administrator

     

    Job Summary:

    Provide direct casework and counseling services to patient/family unit. Provide diagnostic evaluation of psychosocial problems and define social service goal for alleviating problems. Serve as part of interdisciplinary team and coordinate patient care with interdisciplinary team.

    Duties And Responsibilities:

    • Ability to perform a psychosocial assessment and reassessment on all terminally ill patients.  This includes pediatric, adolescent, geriatric and the general patient population.
    • Answers questions and offers emotional support to patient and family.
    • Demonstrates knowledge of hospice social work.
    • Treats patients and their families with respect and dignity.  Identifies and addresses psychosocial needs of patients and family.
    • Communicates professionally with patient/family and involves patient/family in the formation of the plan of care.
    • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.  Supports and maintains a culture of safety and quality.
    • Manages and operates equipment safely and correctly.
    • Demonstrates an ability to be flexible, organized and function under stressful situations.
    • Maintains a good working relationship both within the department and with other departments.
    • Consults other departments as appropriate to collaborate in patient care and performance improvement activities.
    • Documentation meets current standards and policies.

     

    Professional Requirements:

    • Adheres to dress code, appearance is neat and clean.
    • Completes annual educational requirements.
    • Maintains patient confidentiality at all times.
    • Reports to work on time and as scheduled, completes work within designated time.
    • Works flexible hours including evenings and/or weekends as needed or requested
    • Wears identification while on duty, uses computerized punch time system correctly.
    • Attends Interdisciplinary Group meetings as appropriate.

    Regulatory Requirements:

    • Graduate of a Master’s or Bachelor’s program in social work accredited by the Council on Social Work Education
    •  Current State Social Work Licensure.

     

    Language Skills:

    • Able to communicate effectively in English, both verbally and in writing.
    • Additional languages preferred.
    • Strong written and verbal skills.

    Skills:

  • Hospice Liaison
