Forget Me Not
APEX Legacy Program, Forget Me Not, is offered to affirm and honor the life of an individual for their families. Upon admission to hospice services, each patient and their family is offered the opportunity to create a beautiful keepsake, donated by our agency. There are multiple items to choose from such as a memory pillow or teddy bears, made from ones favorite shirt that was often worn, a story book with memorable pictures and stories written by the patient or letters to their family members, Wisdom hands provided on canvas.
Our Legacy program allows our patients to record their treasured memories, life story or even words of wisdom for those that touched their lives. Once the patient has determined they have provided what they wish, the Apex team works to prepare the legacy of their choice and can be passed onto their family members.
